Sick, appointment and leave
Leave of absence or sick?
Parent/guardian/guardian calls between 8 a.m. and 8:30 a.m. on tel. 0650585559 or 0639457988.
Is your child already at school and wants to go home sick? If so, he/she will first report to the mentor or team leader.
Does your child have an appointment?
For example, with the doctor, dentist, immigration police, the consulate or a lawyer?
Make appointments after school
Check the schedule to what time your child has class.
Can the appointment only take place during school hours?
- Parents/guardians/guardians write a bill with their signature
- Your child will bring the appointment invitation with him to school
- Your child will hand in your bill to the mentor and also show the appointment invitation to the mentor
Leave?
- You download the leave of absence request form here. You fill it in and your child hands the form to the mentor
- Your child will receive a message back with approval or rejection from the mentor
Would you like to request whether your child can take vacation earlier?
You can only apply if:
- One of the parents/caregivers cannot leave during the school vacations due to his/her work
- You are asking to take an earlier vacation for the first time this school year
- Your child will miss up to 10 school days as a result
- The extra vacation days do not fall in the first 2 weeks of classes of the school year
- The extra vacation days do not connect to a school vacation.
You make an application as follows:
- You download the vacation leave request form here. You fill it out and your child submits the form to the mentor.
- Your child will receive a message back with approval or rejection from the mentor.
Guidance and care
At EOA -Velp, as a student you always have a mentor. He or she helps you find your way in school. Sometimes you need extra help. Then there are people with special knowledge to give you that help.
Mentor
Do you have a question or problem? Go to your mentor to talk about it. Together you will find a solution and otherwise your mentor will know who can help you best. Your parents can also go to your mentor if they have questions. Besides your mentor, there are other people at school who help you:
Contact person (confidant)
Sometimes it is difficult to tell your own mentor a problem.
That is why there is a contact person at school. She is willing to listen to any problem and will not tell others. She can (sometimes) give advice on how to deal with problems.

Contact school if you or your parents have questions about:
- placement in the correct class
- preparing your transfer to a new school
School doctor (GGD)
The school doctor checks that you are healthy and that you can participate in sports. She checks that you have had your vaccinations. She talks to your parents/caregivers and also your family doctor if she finds health problems. The school doctor is also called in for absenteeism.
